Services

Heritage Center is a 128-bed facility designed on four levels. The facility is newly decorated to provide a more home-like atmosphere. Single and double rooms are available, each offering a private bathroom. Residents are encouraged to being items from home to decorate and personalize their rooms.

Skilled nursing services are provided 24 hours a day by registered nurses, licensed practical nurses, trained medication assistants, rehabilitation aides and certified nursing assistants. Ongoing training activities for nursing staff are geared toward keeping skills current and to insure residents maintain as much independence as possible. By assigning staff to a primary group of residents, a knowledgeable and trusting relationship is built through regular contact.

Services offered by Heritage Center are:

Traditional (long-term care) services for persons who need assistance with a variety of daily living issues, i.e., walking, using the bathroom, eating, dressing, bathing, taking medications, etc.

Transitions on First focuses on residents who need a concentration of rehabilitation therapy, post surgical/hospitalization recovery time or other medical issues that are temporarily preventing them from being discharged to their homes.

          Therapies are also available in-house through           contracted physical, occupational and                    speech therapists.
