
Social Services
Our Social Service staff is available to assist with medically related
social and emotional needs. Social Services continually monitors
the resident's adjustment, determining which services or supports are
beneficial. Social workers are also advocates for resident's
rights and are available to assist in obtaining needed services and
resources. They also help coordinate discharge plans when necessary.
The following are questions that families may have regarding
nursing home placement:
How much will the nursing home cost?
Cost is determined by the amount of care an individual receives. Monthly
room rates average $4000 to $5000 per month. In addition, residents
are responsible for paying for: doctor visits and subsequent costs, medications,
therapies, phone service and a TV if wanted, etc.
How do we pick a nursing home?
Personally visit the facility to observe interactions between staff
and residents,
odors, and cleanliness of the facility. Make an appointment with
the social worker to discuss more details and bed availability.
Are residents
allowed to leave the building once they've been admitted?
Residents who are able to get in and out of a vehicle continue to enjoy going
for outings with family and friends. Residents are free to come and go
just as if they were in their own home or apartment.
What needs to be done
prior to admission?
A Long Term Care Consultation is required prior to admission to the nursing
home. LTCCs
in Kandiyohi County are completed by Kandiyohi County Family Services. If
the person lives in another county, it is suggested that their county family
services be contacted to ask about the screening. All admissions to
Pleasant View and Heritage Center must also be under the care of a local
physician and
have a current history, physical and orders. Contact Social Services
to obtain more information on how this can be accomplished.
What personal
possessions should be brought to the nursing home?
It is suggested that residents have at least five to seven full changes
of clothing, sleepwear, jackets and gloves, caps appropriate for the season. Toiletries
and grooming supplies should also be included. Personal hygiene items such
as shampoo, deodorant, hand cream, etc. are included in the room rate. Families
are encouraged to personalize a resident's room with family photos, a chair from
home, pictures, etc. The facility also requires copies of Social Security,
Medicare and insurance cards, Power of Attorney or Guardianship/Conservator forms
and Health Care Directives.
